Home Health RN

Overview

As a Home Care RN, you will provide comprehensive care to patients with a new diagnosis, medications, or have been discharged from ER, require respiratory treatment or wound care. During home visits, you will ensure patients are progressing with their care plan, provide education related to their disease process, and assess physical and psychological needs to improve quality of life. When it comes time to discharge, you will coordinate resources and educate patient and patient’s family.

  • Full Time Days (Fri – Mon | 10 hour shifts) may include rotating evening call
  • Applicable Differential/On-call Pay

Qualifications

  • Education: ASN or BSN Degree
  • Experience: 1 year of RN experience; Hospice or Acute Care experience helpful
  • Licenses/Certifications: Current RN License in the State of Practice; Current Basic Life Support (BLS) Certification
  • Valid Driver’s License and proof of Auto Insurance
