Pay: $76,000 to $118,000 per year
Shift: Full-Time, Day Shift (8 hours)

This role will focus on helping patients transition smoothly through the continuum of care while collaborating with interdisciplinary teams.

Job Description:

As a Care Manager MSW, you will be responsible for providing psychosocial interventions to remove barriers that may affect a patient’s safe and timely transition through their care journey. Your role will involve close collaboration with healthcare providers, patients, families, and community resources to ensure comprehensive care and enhance patient satisfaction.

The MSW will work within an interdisciplinary team to address the psychosocial and socioeconomic needs of patients, ensuring that they receive the best possible outcomes. You will have the opportunity to make a real impact on patients’ lives by removing obstacles to their recovery and discharge process.

Key Responsibilities:
  • Collaborate with the healthcare team to provide psychosocial interventions and coordinate discharge planning.
  • Address patient and family needs, ensuring comprehensive care throughout the continuum of care.
  • Work closely with patients, hospital staff, physicians, and community liaisons to ensure successful care transitions.
  • Assist in managing the behavioral health needs of patients and contribute to discharge planning.

Requirements: Mandatory Qualifications:
  • Master’s Degree in Social Work (MSW).
  • 1-3 years of recent experience in an acute care inpatient setting.
  • 6 months of advanced communication skills to interact effectively with patients, hospital staff, physicians, and the community.
  • Proficiency in computer literacy as required for the role.
Preferred Qualifications:
  • 6 months experience in care coordination and discharge planning.
  • Familiarity with community resources for patient support and social services.
