Travel Physical Therapist (PT) – Skilled Nursing & Rehabilitation

Schedule: 5 Days Per Week | Tuesday–Saturday or Sunday–Thursday Schedule

Position Overview

We are seeking a compassionate and motivated Physical Therapist for a 13-week travel assignment in Hawaii. This position primarily focuses on Skilled Nursing Facility (SNF) and inpatient rehabilitation patients, with occasional outpatient therapy responsibilities. The ideal candidate will be experienced in evaluating and treating adult patients with a variety of rehabilitation needs while collaborating with an interdisciplinary healthcare team to improve patient mobility, function, and quality of life.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.
• Develop and implement individualized treatment plans based on patient goals and clinical findings.
• Provide rehabilitation services for patients recovering from illness, injury, surgery, and chronic conditions.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients, families, and caregivers on therapeutic exercises and home programs.
• Maintain accurate and timely documentation within the electronic medical record system.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to optimize patient outcomes.
• Participate in discharge planning and continuity of care initiatives.

Qualifications

Required

• Graduate of an accredited Physical Therapy program.
• Active Hawaii Physical Therapist License by assignment start date.
• Current BLS Certification.
• Minimum 6 months of experience in at least one of the following settings:
  • Skilled Nursing Facility (SNF)
  • Acute Care
  • Rehabilitation Hospital
  • Outpatient Rehabilitation
• Experience treating adult patient populations.

Preferred

• Skilled Nursing Facility experience.
• Inpatient Rehabilitation experience.
• Experience working with geriatric populations.
• Strong interdisciplinary communication skills.
