Registered Nurse (RN) – Home Health

Position Overview

We are hiring a dedicated Registered Nurse (RN) for a Home Health opportunity in Berwick, Pennsylvania. This position is ideal for an RN who enjoys providing one-on-one patient care and helping patients maintain their health, safety, and independence at home.

Key Duties

• Conduct comprehensive nursing assessments during home visits
• Deliver skilled nursing services according to physician orders and established care plans
• Monitor vital signs, symptoms, medications, and overall patient progress
• Provide wound care, medication management, and other skilled nursing services as appropriate
• Educate patients and caregivers on disease management and home care
• Identify and report significant changes in patient condition
• Coordinate with physicians, therapists, case managers, and other members of the healthcare team
• Complete clinical documentation accurately and within required timelines
• Maintain compliance with applicable clinical and safety standards

Required Qualifications

• Active RN license or eligibility for appropriate Pennsylvania licensure
• Graduate of an accredited School of Nursing
• NCLEX-RN passed
• Current BLS/CPR certification
• Strong clinical assessment and communication skills
• Ability to manage patient visits independently
• Previous nursing experience preferred; Home Health experience is a plus

Preferred

• Bachelor of Science in Nursing (BSN)
• Home Health, Med-Surg, Rehabilitation, or Skilled Nursing experience
• Experience with electronic medical records and home health documentation
