Overview
We are seeking a dedicated and skilled Physical Therapist to join our team. In this role, you will be responsible for assessing, planning, and implementing individualized treatment programs for patients to enhance their physical function and overall well-being. You will work collaboratively with a multidisciplinary team in various settings, including clinics and hospitals, to provide high-quality care to patients of all ages, with a focus on geriatrics.
Responsibilities
- Conduct thorough assessments of patients’ physical abilities and limitations.
- Develop and implement personalized care plans based on individual patient needs.
- Utilize various therapeutic techniques, including trigger point therapy, to facilitate recovery.
- Educate patients and their families about treatment plans, exercises, and self-care strategies.
- Monitor patient progress and adjust treatment plans as necessary.
- Maintain accurate documentation of patient evaluations, treatment sessions, and progress notes.
- Collaborate with other healthcare professionals to ensure comprehensive care delivery.
- Participate in discharge planning to ensure continuity of care post-treatment.
Experience
- Strong knowledge of anatomy and physiology relevant to physical therapy practices.
- Familiarity with medical terminology used in clinical settings.
- Experience in developing care plans tailored to individual patient needs.
- Background in geriatrics is preferred but not mandatory.
- Previous experience in a clinic or hospital setting is advantageous.
- Ability to effectively communicate with patients, families, and healthcare teams.
Required Qualifications:
- Graduate of an accredited Physical Therapist program.
- Current licensure as a Physical Therapist (PT).
- Basic Life Support (BLS) certification through the American Heart Association (can be obtained upon hire).
- Ability to lift, push, pull, and carry objects over 75 lbs and perform physically demanding tasks.
