Physical Therapist (PT)

Job Type: Travel Contract (13 Weeks)
Setting: Skilled Nursing Facility (SNF)

Job Summary

We are seeking a motivated and compassionate Physical Therapist (PT) to join a Skilled Nursing Facility in Salinas, California. The PT will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and overall functional independence.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide physical therapy services to residents in a Skilled Nursing Facility setting
• Implement evidence-based interventions to improve mobility and functional outcomes
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals
• Educate patients and caregivers on treatment goals and home exercise programs
• Maintain accurate and timely documentation in Net Health EMR
• Ensure compliance with facility policies and regulatory standards
• Participate in interdisciplinary team meetings and care planning

Requirements

• Active California Physical Therapist License
• Current BLS Certification
• Skilled Nursing Facility (SNF) experience preferred
• New graduates and therapists with 0–1 year of experience are encouraged to apply (mentorship available)
• Strong documentation and time-management skills
• Excellent communication and patient care skills
• Flexible and collaborative approach to patient care
